Prism Optical Chicago COVID-19 Statement                                                        Updated: March 30, 2023

Prism Optical Chicago is a small medical facility and masks are no longer required. Appointments are available for those who would still prefer masks be worn.

Vaccines are widely available. All employees at Prism Optical are fully vaccinated with busters. We are still monitoring COVID-19 and emerging mutations as well as following all recommended health guidelines recommended by our local government and CDC.

At this time Prism Optical Chicago is open normal business hours but appointments are still available for one on one service. We also have availability before and after normal hours if needed. We know that your vision is important so if you’re running low on contact lenses or you’ve lost or broken your glasses we are here to help, just call, email or text to set up your appointment. Our one on one appointments mean we are doing everything to keep our patients safe and give them the attention and service they need.

We are providing routine needs such as new glasses, sunglasses or contact lenses, and we are taking appointments for eye exam visits.  At this time we are still limited on eye exam times so we ask you to arrive 10 to 15 minutes early and be prepared to wait in your car if necessary. On the day of your eye exam we may not have time to look for new glasses so we ask that you schedule a separate time for that so we can give you the attention you deserve when looking for a new frame.

Before entering the store, if any of the below are applicable, we respectfully require you to reschedule your appointment for a later date.

-You have tested positive or had close contact with a person who tested positive for COVID-19 or show any symptoms in the last 5 days.
-You have a severe headache with fever, loss of taste or smell, feeling confused, having a hard time staying focused, having unexplained memory loss, any lower respiratory symptoms or symptoms of the flu. If this is the case, we recommend you contact your primary care physician.

-It is mandatory that you limit the number of people entering our shop to patient and essential guardian if necessary.

Eye exams are available at this time, so please call or fill out your information and submit it here. Only the patient and essential guardian will be allowed in the store for the safety of our customers and staff.
